How do I get a certificate of good moral?

Im applying at a university in philippines and one of the requirements is a certificate of good moral. How do I get a form/template? do i make it myself, get it from my highschool (my highschool doesnt have one), get it from the university I'm applying to?

1. Get it from the principal of the high school you graduated! They will write the letter themselves. Or you can get one from the Priest or Minister of your religion!
2. Certification of good moral character are provided from your former school guidance counselor, from your municipality, yup from your city where you reside. I guess there is a CITY HALL in your area unless you live in a forest or a cave. Your HS doesn't have a form for this? How come, you are their former student obviously they know who you are more than anybody else. Then get it from your city hall, do not tell me again your city do not have such form. Why you are not a community person in your area? They do not know you? I do not think religious groups have anything to do with such certification of good moral character. Even if you are a member, religious association have no barring with such public documents.
3. just ask for it in your school registrar.. they will give you a letter and that letter will be the one you use.
