DH Templates

I have a small business and i was wondering if theyre is anywhere i can get a template of a group certificate?

this is the first year we are doing group certificates and were just unsure on how to go about it. we have the quickbooks programme but it doesn't seem to have it on there. we've been searching the web but cant find any templates. not quite sure if were searching the right thing tho. are they still called group certificates or have they renamed them?? Thanks in advance xo

1. PAYG payment summary - individual non-business (group certificates) are available from the ATO. They are not computer generated but manually filled in.
