Avery 5195 template not in MS Word template list?

I just bought Avery labels Template 5195, and it isn't in my MS Word list. I can find templates that already have info in them, but I just want the blank labels so I can fill them in for myself. Where can I get this?

1. Get the Word template directly from Avery's website, by reference number (5195).
2. Sometimes the Avery labels package has a note by the label number indicating an alternative label number to use in such a situation. Hope that helps.
