Writing a letter to confirm change of name..?

I am sending highschool transcripts to a university -- my transcripts have my mother's married name on them (step dad) .. and they are now divorced. I do not speak to my parents, know where they are, and have no proof of this change what so ever. Havn't heard from anyone in 6 years. Anyway.. :D .. Point being, I have no official documentation of marriage/divorce or anything. All I have is my birth certificate with my legal name on it, which is what I now and have been using for the past 6 years. My program coordinator has asked me to write a letter explaining the situation.. but I can not find any templates. Any help would be appreciated. I basically need to explain why I have gone under two different names, and also that there is no official documentation to support it. I have to explain that in a professional and detailed manner, without of course getting too carried away in the drama that is life. Thanks peeps!

  1. Why can't you go to the DMV or RMV and get a vaild license or id card. You have your birth certificate, you could also take that to your local social security office to get a card printed up for you. Get some other legal documentation so you don't have to explain your private situation to a bunch of strangers.
