Does anyone have a letter template for returning to work after maternity leave?

Just looking for any templates to write to my employer of my intention of returning to work.

1. I never had to write a letter for my intention of coming back to work. Your HR will fill out paperwork for your leave ( i gave them my estimated return date) and FMLA protects your job so it will be there when you return.
2. I never had to write a letter. I just kept communication with my boss & the HR dept. Everything was either done via email or phone.
3. I just wrote a simple letter stating the date I will be returning, if you've took your full allowance then they should be expecting you back anyway, so you'd just be informing them you know.
