I need a template for a "certificate of ownership" for my puppies.?

I am selling two of our puppies to a canadian, and in order to get the puppies over the border from the US into Canada I need a "certificate of sale" or a "certificate of ownership" for the buyers. Does anyone know where I can get an official looking template in word or excel? I am also looking for a vaccination form to show the shots they have received, which we are giving them ourselves. Thank you!

  2. You can get a Certifcate of sale form at any Staples or Office Depot. The other, for vaccines requires a Vet's signature.
  3. What? I have brought dogs back and forth across the Canadian border countless times and have never had to produce anything. If they want paperwork, your signed contract will be enough to prove ownership. Make sure you put their microchip numbers on them. The vet will provide you with a vaccination form and health certificate, which you should be providing for all puppy buyers.
  4. They need an official vet health certificate to transport them over the border. Do-it-yourself doesn't work in this situation for the shots.
  5. If this is required, it doesn't have to be an official looking document. Any written document is legal when signed, etc. When it comes to the shots, you'll need the stickers off of the vaccinations, and then visit the vet for a signed bill of health. I however, have not been required to provide such documents crossing into Canada. All I need were licenses and vaccination proof.
