Specific Business Letter Template?

Hi I'm trying to update companies customer database to make sure all the contact info, etc. is current. Here are the issues that need to be stated: 1. Telling customer about our intent to update 2. Fill-out form (which will be attached) and mail it back Something along those lines. I BEG you, PLEASE HELP ME!!!!

1. It's not really clear what you're asking for, but I'll take a guess. You're wanting to be able to e-mail your customers a form that they will fill out and send back to you and it will automatically be plugged into your database? I believe Adobe Professional has the function to create forms that folks fill out that can collect and compile all the information. I think what you really want though is a web page that people could go to to fill in this info. When web pages collect info, they collect it in a database. You could then connect that database to your database and import the information. You could use a survey maker website to create a "survey" and the survey questions would be the info you want to collect from your customers. Oh wait... I just realized that you're asking for a template to e-mail out to folks that would just talk about your intent to collect this info. I think the first two options I suggested are better choices.
