Template for Invoice?

I work as a freelance web developer. What's a good Template for an Invoice? I don't want to include my address or phone number, just my email address. I don't have a company name. All I need to list is Day, Hours Worked, and What was Done. Should I do this in Photoshop (pdf files are huge, but uneditable), or Word (doc files are small, completely editable)? Should it just be text, or should I try to make it "classy" somehow? -I do have Microsoft Office, but Microsoft says it's an illegial copy. (It would appear that the IT department has screwed me once again.) -Dude, you have a lot of tutorials on your site. Could I make a suggestion: Include a meta description so Google can index you properly. Find a SEO tutorial and optimize your site (for google). Get rid of the frames; they make it impossible to bookmark pages on your site and they're just plain annoying. Last but probably most important, redesign. Two columns was fine back in the day, but you gotta make your site jump out at people. You've got a great product (tutorials), but you gotta market it properly too.
