How to create my index page layout from scratch?

Do I need to create the tables , menue etc,, from scratch? Using what? html? CSS? I am use to have a template then work on it. I never start an index blank page. Please advise me what I need to know in order to succeed? Tutorials. tools etc.. Thx.
