How do i write a return notice from maternity leave?

I have already contacted Human Resources about my date of return. However, they want me to email my boss in writing about my start time, position, date , etc. What do i put and does anyone know where i could get a letter template (free) for it.

1. I couldn't find a template but it should be professional and look like this.. Today's date Your name Address City, State, Zip Code Dear {Name of your boss}, {In here you should say that you are planning to return to work on a certain date. Also you should put in that you are ready and excited to return back to work.} Sincerely, {Your name}
2. don't think you really need a template. just a standard letter saying Dear Mr. Whatsit at the beginning with a couple paragraphs giving the info, then Yours sincerely etc. the info is exactly what they said: start date, position etc etc good luck.

4. Dear Boss (insert name), I am ready to return to my position as (insert) at (name of Co. here) from maternity leave on (insert date & time here). I have already notified Human Resources of the date of my return and now I am notifying you in writing as they requested. Sincerely, (your name, address, phone # here)
