Baby Birth Certificate?

My friend recently delivered a baby girl. I want to give her one of those cute personalized baby birth certificates. Where do I get it? Can I get it online? Any place that also offers free templates for the same? Uh...just wanted to let u know all, that I did google it...but I was not getting many good sites...

1. Definitely look online...in the hospital where I delivered they gave us a form for one from a different company that we could fill out when they came in and took our daughter's birth picture.
