Mad panic from a friend for a handicap certificate ahead of a golf holiday. Any idea on getting a template?

- A friend of mine went to a web site of some American golf club, down loaded the logo, cut and pasted it on a card size template he made on pagemaker or some other desktop publishing software and TA DA! he had a cert. He had it laminated (it's about the same size as a credit card) and it looks pretty real. Real enough so no one has challanged it. This handicap certificate stuff is a pain for green fee players who play a variety of pay and play courses and don't want to pay for club membership. My friend put his real estimated handicap (14) on the card and everyone was happy. If the aim of having a handicap cert is to show you actually play golf at a decent level, I have no problem with his fudging a wee bit.
