DH Templates

I need a template for an official looking Certificate/Diploma/Completion Certificate?

I just completed a one year fellowship in my work place. I want to fill out and print an official looking "graduation" diploma/certificate type of thing, and have my fellowship director sign it before I move. Does anyone here any an offiical look template that I could get/download to use? Thanks.
