Free birth certificate template?

This is for my daughters school project and I cannot find a simple free template...I'm not creative enough to create one in OpenOffice. I have seen a few pay sites but for a simple on time deal I need a free site. Any help is much appreciated.

1. While looking for the same thing for my 4th grader's project I stumbled upon your question. I kept looking and found what I am going to go with. Go to http://www.dyetub.com/certificates/ Pick a category (we're using 2 from the first group), pick a color and style and customize every line, then print. We need a birth and death certificate... This works for me!! I just had to come back and let you in on what I found. Good luck! :)
