HELP!!! REFERENCE letter for EXCHANGE STUDY?

I asked my lecturer to write a reference letter for me for my exchange study in a university, but he is too lazy to write it, so he asked me to write it for him. I have no prior experience on writing a reference letter. Don't even have a clue how to start writing it. The letter needs to include applicant's suitability for the Exchange Program considering such things as organisational ability, adaptability/versatility, maturity and independence. Also give appraisal of the applicant's communication skills(written, verbal, comprehension and interpersonal skills) Could anyone be so kind to write me a exchange letter or maybe provide me with a template or some good quotes? Any help is appreciated!
