letter of reference for my business?

How do I request a letter of reference from a client? Any formal business templates or suggestions to make it sound good?

1. An original letter on client's letterhead or personal stationery would be very impressive. If that is too much to ask (try anyway!) then you might come up with a checklist that they can rate you from 1-4 (excellent, good, fair, poor) so that the client only has to check boxes and sign his name. Was xyz company courteous? Was the rep dressed professionally? Were the products delivered within the time promised? And so on...
