How do i write an early return notice for maternity leave?

i have to write a letter to notify my employer of my intention to return to work early. I have to give 8 weeks notice but dont know where to start. What do i put and does anyone know where i could get a letter template (free) for it.

1. Talk to your boss. Surely they have something that you can fill out. Also, you need to make sure that they will allow you to return early. Most places, you are not allowed to return to work until you have gotten the clear from your doctor at your 6 week post-partum checkup. Other than that, I can't imagine it needs to be anything too impressive. Just a simple "Assuming I have no complications during delivery and assuming that the baby is born healthy, I will be returning to work on such and such date. I will notify you ASAP with any changes."
2. Have you spoken to your boss about it? They may be able to tell you what needs to be included. Or you could just write something along the lines of. I am writing to inform you that I will be returning to work on blah blah date.
