Need a template for official letter to Dean.?

Please write a letter for me for the following request: I was attend a BUSINESS study class ( Code BTM 7102) and get a grade of B+ , after few month later the subject code is change to another code call ESM 7102 and I wrongly register in the WEB system,after end of semester, i get the result of F (fail) because I didn't know I registed the subjest and never submit any assignmnet and also the exam.. So I need write a official letter to my dean and request to withdraw the subject of ESM7012 and restore the original BTM 7102 result which is B+。 Subject of the officail letter should be : Request withdraw the ESM 7102 and restore the BTM 7102 result.
